How old is the machine?? If it seems to be clean, let's make sure that it is clean. Take off the cover and make sure there's no lint or anything else trapped between the wheel and the metal. Possible fixes; Clean the wheel sensor. Make sure that the thread path is correct and you don't have the thread being held up somewhere prior to the wheel sensor. If it turns out that the wheel sensor is moving freely but not picking up the thread as it goes around once, try wraping the thread twice on the wheel sensor (do this anyway if it's an old machine). Do tensions have anything to do w/ it? Not likely. You can pull thread from the needle and see if it feels any different than the other needles.. better yet, do a yo-yo test on your bobbin and do an H test on your machine to verify that the tensions are set correctly. My best guess is that your thread path is incorrect and the only sure way to fix it will be to start over from the cone. After you do that, if it still happens, wrap the thread twice around the wheel senson. Try that first, if it doesn't work, try 800-456-3727 and ask for tech support.
