If the guy does that much with you a year, I would just have a sit down with him and let him know you did not appreciate all the trouble you had to go through to fix the mess and if he would have just asked you would have pointed him to them so he could setup an account. Doing that much with you I would say he is a contract or trade customer anyway and I would let him provide the shirts if he wants, just let him know about spoilage.
